This cluster-randomized trial evaluated the effectiveness of a standardized handoff process at preventing adverse events in hospitalized medical patients. Compared to usual handoff processes, the structured tool did not achieve reductions in adverse events or other safety metrics, although the completeness of handoffs did improve.
