This study applied a novel theoretical framework to medication errors involving chemotherapy drugs to characterize underlying causes. The authors found that working conditions often contributed to medication errors, consistent with prior studies of high-risk medications. These results suggest that vulnerabilities remain despite longstanding focus on safe administration of chemotherapy.
