Root cause analysis is a strategy to identify and reduce risks, but there are concerns regarding its effectiveness. This improvement report discusses the development of a program that used simulation to enhance trainees' root cause analysis skills. The students involved in the program gained confidence in their ability to recognize, identify, and report adverse events and near misses.
