Errors, near misses and adverse events in the emergency department: what can patients tell us?

This study examined patient reports of adverse events, building on past research that used similar information to measure safety. Investigators interviewed patients within 24 hours of emergency department discharge and found that they identified a number of events not captured in the medical record.
