This article discusses a working group dedicated to improving safety culture as a means to reduce adverse drug events. While acknowledging the importance of existing pharmacovigilance strategies to ensure medication safety, the authors advocate for greater focus on systemic issues and highlight the importance of both these approaches being used in a collaborative, rather than independent, fashion.
