Formal root cause analysis (RCA) training conducted by the United Kingdom's National Patient Safety Agency improved participants' understanding of how to properly conduct an RCA. However, a follow-up survey several months later revealed that organizational barriers frequently limited the implementation and dissemination of RCA results.
