This narrative review summarizes an in-depth report on measuring safety and monitoring improvements. The authors outline five considerations identified as key elements of enhancing organizational safety: rates of previous patient harm, reliability of systems and processes, continuous safety monitoring, preparedness for future hazards, and learning and responding to reported safety information.
