Errors of omission can lead to delayed or missed diagnosis. In this qualitative study, researchers interviewed 26 primary care providers to characterize errors of omission occurring in primary care. They found that omitting patient education, follow-up, emotional support, and consideration of mental health needs were commonly cited. Providers identified lack of time, emergencies, unplanned visits, and administrative workload as contributing factors.
