Preventing medication errors in long-term care: results and evaluation of a large scale web-based error reporting system.
Medication errors are a common, costly, and dangerous problem for residents of long-term care facilities. This study reports the initial results of an Internet-based medication error reporting system for nursing homes in North Carolina. The system was established in response to state legislation mandating error reporting. More than 2700 errors were reported in the first year, primarily involving errors in medication administration, and frequently involving known high-risk medications such as sedatives, pain medications, and anticoagulants. Participants found the system to be easy to use.
