Patient Safety: Achieving a New Standard for Care.

This report details findings and recommendations related to establishing a national health information infrastructure. The Institute of Medicine committee recommends plans for future work in setting data standards. The committee also recommends that all health care organizations establish a patient safety program and provides a description of such a program. Finally, the committee recommends the development of a common system for reporting adverse events.
