Diagnostic errors are increasingly recognized as a critical safety issue. This survey of physicians found that cognitive error was the most common contributor to missed and delayed diagnosis, followed by inadequate staffing and excessive workload. The authors expressed concern about the low frequency of perceived diagnostic errors and recommend further study.
