Consistent with findings from prior studies, this retrospective chart review of incident reports at a children's hospital found that communication lapses and handoffs contributed to medication errors, which often led to increased need for monitoring but no harm. A recent AHRQ WebM&M commentary discusses a pediatric medication error.
