Incident reporting is widely utilized to detect patient harm, but underreporting is a well-known challenge, especially among physicians. In this mixed-methods study across 11 hospitals in England, researchers surveyed physicians on incident reporting and found that incident reporting still requires improvement. Participants cited time, the reporting process, culture, organizational issues, and inadequate feedback as barriers to reporting.
