The first version of this now annual report on the safety of hospitalized Medicare patients received substantial media coverage when it appeared in 2004. The report used the Patient Safety Indicators developed for the Agency for Healthcare Research and Quality, but it did not necessarily apply them in the recommended manner. Organizations reviewing the report’s findings should bear in mind these potential methodologic shortcomings and the fact that neither report has undergone external peer review.
