This chart review study included patients seen in emergency departments (EDs) who had evidence of pre-existing adverse drug events (ADEs). Researchers found that ED physicians failed to attribute patients' conditions to medication effects in the majority of cases. This under-recognition of ADEs may lead to continued use of an unsafe medication with ongoing or increasing harm.
