Root cause analysis (RCA) is frequently used by health care organizations to understand factors contributing to adverse events. This qualitative analysis explored RCA team members’ perspectives about the RCA process and implementation of RCA findings into clinical practice. Participants expressed perceived value in reviewing adverse events using RCA but expressed concerns regarding workload and lack of impact.
