Despite two decades of broad-based initiatives to improve the quality and safety of health care, progress has been slow. This commentary discusses how embedding preoccupation with failure and building resiliency in health care can motivate improvements. The authors suggest that these elements of high reliability and error management theories could drive and sustain progress in safety efforts.
