While several instances of serious errors in the administration of inpatient chemotherapy have garnered wide publicity, prior research has also found that outpatient oral chemotherapy medication errors are also relatively common. This study surveyed pharmacy directors at U.S. cancer centers regarding their prescribing and safety practices for oral chemotherapy. Few centers had formal safety protocols, and rarely required practices that are considered standard for inpatient chemotherapy, such as documentation of the patient's diagnosis or double-checking of the prescription by a second clinician. A prior study explores the epidemiology of outpatient chemotherapy errors and suggests preventive strategies.
