Maternal care processes are prone to a variety of failures that contribute to patient harm. This initiative seeks to review clinical practices and make recommendations reduce risks to material and infant safety. Initiatives include the Confidential Enquiries into Maternal and Infant Deaths program which produces investigations into incidents of maternal harm.
