The development, implementation, and use of health information technology (IT) requires cross-industry cooperation and accountability to avoid unintended consequences that contribute to care failures. This editorial challenges health care to share the responsibility for the safe use of health IT and suggests a collaborative approach to guide design of policy, process, and measures to ensure the reliability of health IT.
