Frequency of and risk factors for preventable medication-related hospital admissions in the Netherlands.
This multi-center Hospital Admissions Related to Medication (HARM) Study analyzed 13,000 unplanned hospital admissions, approximately 6% (714) of which were medication related, and nearly half of those (332) were potentially preventable. The study further elucidates patient factors (e.g., impaired cognition, co-morbidities) associated with preventable medication-related admissions.
