To avoid excessive dosing of chemotherapeutic agents, standardized dose limits must be agreed upon and incorporated into systems such as computerized provider order entry. This study evaluated more than 280,000 drug administrations from 82 oncology practices to determine the range of user-defined dose limits. Investigators identified a dose limit defined by body surface that was not exceeded by 97% of providers. They suggest that the complexity in prescribing and delivery chemotherapeutic agents can benefit from implementation of such dose limits, particularly to avoid errors associated with necessary weight-based calculations.
